J. Have only personnel licensed by the Arkansas State Health Department accomplish <br /> repairs, installation, maintenance and testing of backflow prevention assemblies. <br /> Assemblies for fire protection lines tapped from a domestic service line shall meet <br /> the same criteria. Repairs shall be made immediately upon notification by the <br /> tester. Overhaul shall be accomplished at intervals not to exceed five (5) years. <br /> Section 5.1 BACKFLOW PREVENTION ASSEMBLIES shall be modified to read as <br /> follows: <br /> Only those assemblies that are approved by BWU shall be used for backflow <br /> prevention in water service lines. <br /> A. Approved Assembly: Assemblies that have been tested and approved by the <br /> Foundation for Cross Connection Control and Hydraulic Research at the <br /> University of Southern California and certified by the Arkansas Health Department <br /> are approved by BWU. <br /> B. The following information shall be distinctly marked on every RPZA, DCVA, <br /> PVBA, DDCVA and DRPZA by cast in the metal, stamped in the metal, or <br /> stamped on a brass or stainless steel nameplate permanently affixed to the <br /> assembly: <br /> • (1) name or trademark, <br /> (2) type (RPZA, PVBA, DDCVA, DRPZA, DCVA), <br /> (3) size, <br /> (4) model number, <br /> (5) direction of flow (indicated by an arrow), <br /> (6) serial number, <br /> (7) maximum working water pressure, and <br /> (8) maximum water temperature for which designed (designate degree F or <br /> degree C). <br /> C. Every RPZA, DCVA, PVBA, DDCVA and DRPZA shall be shipped from the <br /> manufacture completely assembled. <br /> 11 <br /> I <br />
