(b) Application form. A subdivision application form providing the following information shall be <br /> completed by the applicant and submitted to the planning commission: <br /> (1) Proposed name of subdivision. <br /> (2) Proposed type of subdivision. <br /> (3) Name and address of owner of record. <br /> (4) Source of title giving deed record book and page number or instrument number. <br /> (5) Name and address of subdivider. <br /> (6) Linear feet in streets. <br /> (7) Average size of lots and minimum lot size. <br /> (8) Number of lots and lots per gross acre. <br /> (9) Location of the tract by legal description giving acreage to the nearest one-tenth of an acre. <br /> (10) Existing and proposed covenants and restrictions. <br /> (11) Proposed open space. <br /> (12) Source of water supply. <br /> (13) Where wastewater disposal is to be accomplished by extending wastewater utility facilities, <br /> this circumstance shall be indicated. In those instances where extraordinary wastewater <br /> disposal methods (any method other than a public wastewater utility)are proposed in lieu of <br /> extending public wastewater systems, detailed information shall accompany the plat. <br /> (14) Such further information as the subdivider wishes to bring to the attention of the planning <br /> commission. <br /> (15) Submission of necessary fees as described in Section 2.6 Filing Fees. <br /> (c) Vicinity map. The vicinity map shall cover an area within a radius of one-half mile of the proposed <br /> subdivision at a scale of one inch equals 2,000 feet. The drawing shall generally locate arterial <br /> streets, highways, section lines, railroads, schools, parks and other significant community facilities, <br /> and if possible shall be incorporated on the preliminary plat. <br /> (d) Preliminary plat. Fifteen black or blue line prints of the preliminary plat, clearly and legibly drawn, <br /> shall be submitted on white paper no larger than 24 inches by 36 inches. Extra-large plats may be <br /> submitted on more than one conforming sheet. Plat scale shall be one inch equals 50 feet for plats <br /> up to and including ten acres and one inch equals 100 feet for plats larger than ten acres, except <br /> where a smaller scale may be deemed appropriate by the staff. The preliminary plat shall also be <br /> submitted in an electronic format, on medium, compatible with equipment available to the City. <br /> • Data shall be an AutoCad file in.dwg format(or such other file format as may be determined by the <br /> city from time to time) containing the information listed below. The City may also require certain <br /> other information to be shown on separate layers to facilitate integration into the City's mapping <br /> system.
