• (iv) Type of assembly date of installation and installation <br /> specifications. <br /> (v) Number of BWU meter, if any. <br /> (vi) Date of initial cross connection survey, survey results and <br /> type of actual or potential hazard. <br /> (vii) Date of initial permit and current permit number. <br /> (viii) Test results before and after repair or maintenance and date <br /> of latest retest. <br /> (ix) Maintenance performed, repairs made, replacement parts, <br /> part numbers and date repairs were made. <br /> (x) Information on backflows through the assembly. <br /> (13) Maintain an inventory of all commercial, institutional and industrial <br /> locations with complete information on cross connection devices or <br /> assemblies installed. <br /> • (14) Install a BWU owned meter on new fire protection assemblies. <br /> (15) Ensure meters on fire protection assemblies are read and consumer advised <br /> of water usage. <br /> (16) Coordinate repairs on any damage resulting from vandalism or flooding of <br /> backflow prevention assemblies. <br /> (17) Ensure a Certified Assembly Repair Technician is responsible for all repairs <br /> performed on an assembly. A Journeyman or Master Plumber's License <br /> shall be required when installing backflow prevention assemblies within the <br /> scope of the plumbing system. <br /> (18) Verify only Certified Assembly Testing Technicians perform testing on <br /> backflow prevention assemblies. <br /> 2.1 POWERS AND AUTHORITIES OF INSPECTORS <br /> A.
