f <br /> (8) Maintain RPZAs and meters for issue to water users needing temporary <br /> ® water service from fire hydrants. <br /> (9) Ensure RPZAs used by customers are tested annually and before issue. <br /> (10) Verify collection of fees for permits and for the use of RPZAs owned by <br /> the BWU. <br /> (11) Verify fire hydrant RPZAs and meters are set and removed by BWU <br /> Personnel. <br /> (12) Submit all required reports, maintain a data base, coordinate with other <br /> agencies to accomplish the purposes set forth in paragraph 1.2 and <br /> maintain the following records: <br /> (a) Master files on customer Cross Connection Tests. <br /> (b) Master files on Cross Connection Permits. <br /> (c) Copies of permits and permit applications. <br /> (d) Copies of lists and summaries supplied to the State Health <br /> • Department. <br /> (e) Number of annual tests conducted on backflow prevention <br /> assemblies. <br /> (f) Number of cross connection control surveys performed. <br /> (g) Total number of each type of backflow prevention assemblies that is <br /> installed. <br /> (h) The following information is required per assembly. DDCVA and <br /> DRPZA are made up of two assemblies, each requiring record data. <br /> (i) Customer's name, mailing address, phone#, contact name, <br /> assembly address, permit # and account number. <br /> (ii) Type installation, problem history, location on property, <br /> installed by, phone# and type of service. <br /> (iii) Name of the manufacturer, model number and serial number <br /> of assembly. <br /> • 6 <br />
