TARDINESS <br /> Tardiness is defined as the failure of an employee to report to his/her work area <br /> ready to start to work at the scheduled work time. <br /> If an employee is going to be late reporting to work, he/she must use the same <br /> reporting procedure detailed in "Reporting Absences" above, and advise the <br /> Supervisor what time he/she expects to be at work. <br /> "Excessive tardiness" is defined as being late on more than five occasions in any <br /> 90-day period. Employees with excessive tardiness will be subject to <br /> progressive disciplinary procedures. <br /> The City of Batesville recognized that occasionally tardiness cannot be avoided. <br /> Each tardiness will be handled on a case-by-case basis, depending on the <br /> circumstances. <br /> H. PREGNANCY LEAVE <br /> It is the City of Batesville's policy to treat pregnancy, childbirth, and related <br /> medical conditions according to the leave requirements outlined in the Family <br /> and Medical Leave Act of 1993. Information of FMLA leave is outlined in <br /> Employee Benefits. <br /> The employee must file a "Leave Request" form with the City Clerk's office. The <br /> employee must use all vacation and sick leave before taking time off without pay. <br /> If additional time off is requested, it will be time off without pay. <br /> Pregnant employees are expected to continue working as close to the anticipated <br /> delivery date as deemed medically appropriate by their physician. <br /> Before returning to work, the employee must furnish a "Physician's Release for <br /> Work" form from her physician stating that she is able to return to work. The <br /> "Leave Request" and the "Physician's Release for Work" forms may be obtained <br /> from the City Clerk's office. <br /> I. MILITARY LEAVE <br /> The City of Batesville will comply with the Uniformed Services Employment and <br /> Reemployment Rights Act of 1994 (USERRA). Employees requesting a military <br /> leave of absence may obtain a copy of the Act or information concerning the Act <br /> from the City Clerk's office. <br /> 12 <br />
